NAFI Connecticut provides programs, consultation, and support services in a variety of settings across Connecticut, New York and Rhode Island. We work with adults, children, and families to increase communication and social skills, improve relationships, increase circles of support, and provide therapeutic services based on best-practice research. The Enhanced Outpatient Services (EOS) Program is exploring candidates for Therapist/ Clinician to provide comprehensive home- and community-based treatment for families, children, and adolescents. Responsible for therapy and/or case management of assigned cases and for serving as the primary contact for family members and others who are involved with clients. Provides trauma-informed clinical services to clients and their families through individual, group, and family therapy, both at the program and in the community, as dictated by program and/or treatment models.
EOS Therapist Overview:
- Gathers, reviews, and assesses necessary information as part of the intake process, including contacting and collaborating with referral sources
- Assesses clients’ risk level and develops/implements and monitors safety plan(s)
- Provides clinical consultation to program staff and other treatment team members
- Maintains regular contact/visits with clients, families, outside collaterals, and stakeholders
- Provides individual, group, and/or family therapy to clients/families in program(s), communities, and/or family homes
- Refers clients and/or families to appropriate community resources
- Maintains professional boundaries with clients, families' employees, stakeholders, and others at all times
- Completes all clinical documentation, including but not limited to assessments, treatment plans, progress notes, and specific evidence-based paperwork as required by licensing/funding source and agency
- Participates in staff training, maintains certifications as appropriate and required, and demonstrates the ability to pass proficiency test(s) as required by program contract and program/agency policy
- Provides on-call coverage on a rotating basis as assigned / as required by program contract/policy
- Provides and/or coordinates safe transportation for clients and families as necessary / required
- Performs crisis prevention and interventions, including de-escalation techniques and/or physical management as needed and allowed by contract and program/agency policy
- Demonstrates computer literacy by utilizing Human Resource Information System (HRIS), Electronic Health Records, Email, and other web-based platforms
- Completes other related duties as assigned
- Performs all duties in accordance with the agency’s policies and procedures

- We welcome candidates who have not yet completed their master’s degree. Ideal applicants will have an MA or MS in Social Work, Family Therapy, Mental Health Counseling, or a related field, or be currently enrolled in such a program.
Salary: $55,000.00 - $68,000.00 / per hour
