Title: Integrated Case Manager
Location: Detroit, MI
Duration: 13 Weeks Contract (Strong possibility of extension)
Shift: Days 8a-4:30p, rotating weekends.
Pay Range: $50 to $55/hr. on W2
Job Description:
- Looking for highly experienced candidates with acute inpatient case management experience
- Training on unit will be minimal - candidates will need to be able to pick up quickly and work efficiently.
Required:
- Minimum 5 years of Inpatient Case Management experience (this does not include telephonic, utilization management, or therapy experience), Strong computer skills, Epic EMR experience, Nursing degree or MSW, BLS certification through AHA, current valid MI RN licensure or LMSW with current valid MI licensure.
Preferred:
- Experience in discharge planning, home health care, rehabilitative medicine, community health, or managed care.
- Typical Case Ratio: 1:21-25 will not meet with each patient, decided based on individual risk factors
Duties:
- The Integrated case Manager for Population Health is an interdependent member of the patient-centered care team or treatment team responsible for the collaborative practice of assessment, planning, facilitation, care coordination, evaluation and advocacy for options and services to meet an individual’s and family’s comprehensive health care needs though communication and available resources to promote patient safety, quality of care and cost effective outcomes.
- Addresses the needs of patients who have experienced a critical event or diagnosis that requires complex management strategies and the extensive use of resources to optimize health outcomes along the care continuum.
- Provides services to patients from ambulatory, inpatient or health plan settings.
Salary : $50 - $55
