Position Overview
This is a part-time 1099 role with opportunity to expand.
The physician will serve as the clinical lead (“point person”) across two Skilled Nursing Facilities, working closely with Nurse Practitioners.
Typical short-term rehab (STR) population:
- Approximately 20–30% of total facility census
- May vary depending on building size and referral patterns
Core Responsibilities
- Perform initial PM&R evaluation sweeps for short-term rehab (STR) patients
- Develop and implement rehab-focused care plans
- Collaborate with therapy teams and facility staff
- Train and support Nurse Practitioners during initial ramp-up
- Provide ongoing oversight for complex patients
- Ensure high-quality, compliant documentation
Compensation
We offer a clear, structured compensation model that separates the initial facility buildout from ongoing responsibilities.
Month 1 – Facility Launch (Buildout Phase)
Compensation is tied to completing the initial setup of each facility:
- $4,000 per facility
- (Initial evaluations care plan development for STR patients)
- $1,000 per facility completion bonus
(Paid upon completion of evaluations and care plan implementation)
- $2,000 one-time training bonus per Nurse Practitioner
(Includes onboarding, training, and oversight during the initial ramp-up period)Typical 1st Month Ramp-Up and Evaluations Total (2 facilities, 1 NP):
Approximately $11,000 – $13,000
Month 2 (Ongoing Model)
- $2,000/month base
- (Covers ongoing oversight of up to 2 Nurse Practitioners across 2 facilities)
- $400–$500 per in-person facility visitTypical Monthly Range:
$3,000 – $6,000 , depending on level of involvement
Focus shifts to:
- Oversight of NPs
- Management of complex patients
- Strategic clinical involvement rather than volume
Qualifications
- MD or DO required
- Board Certified or Board Eligible in Physical Medicine & Rehabilitation (PM&R)
- Active Virginia license (or ability to obtain)
- SNF/post-acute experience preferred but not required
