What are the responsibilities and job description for the Program Coordinator/Case Manager position at My Day Counts?
Responsibilities:
- As Program Coordinator, you will be responsible for maintaining communication between Program Training Instructors, RCOC, participants and stakeholders.
- You will assist the Program Director in the development of program goals with support to the team as well as the participants that we serve.
- You will attend the semi-annual and annual meetings with RCOC to report participants’ behaviors and assist in the development in their person-centered-planning (PCP).
- Ultimately, you will directly support the Program Director in the daily maintenance and implication of services for the program.
Duties will include, but are not limited to:
Develop day program goals for each participant’s Individual Service Plan
Produce all required documentation in a professional and timely manner
Support and assist Program Instructors in their delivery of program services
Assign daily and monthly duties and activities
Participant conflict resolution and track behaviors such as bullying
Report all participant’s emergencies, behaviors and incidents and send all reports, SIR’s, and Aps to RCOC
Case record review/ maintenance/ and updates: assessments, acknowledgements and reviews
Attend Semi-Annual and Annual IPP meetings
Advocate for services and supports that will enhance the quality of life for participants
Maintain positive relations with all stakeholders while professionally representing My Day Counts
Qualifications: A.A. Degree, or comparable experience working with developmentally disabled adults and in a leadership role.
- Excellent written and oral communications skills.
- Competent in Microsoft Word.
