Position Overview
Under the direct supervision of the Patient Care Manager, the Care Coordinator works with patients who have been referred for assistance in accessing and utilizing resources due to their complex chronic health conditions and/or social needs. The Care Coordinator will also review reports and track data for Emergency Department usage and ADT.
Essential Duties and Responsibilities
- Understands the principles of a Patient Centered Medical Home.
- Monitors and tracks Emergency Department usage and provides education to patients with high Emergency Department utilization.
- Identifies the target high-risk population within the practice through referrals from providers, care managers, and use of risk stratification tools. This includes patients with social, behavioral, and/or healthcare needs.
- Monitors and evaluates the health, educational and psychosocial needs of the patient and family longitudinally.
- Coordinates care between patients, providers, and care managers, to enhance communication and maintain continuity of care.
- Consults with multidisciplinary team members to coordinate services for patient education, preventive care, social needs and disease management.
- Collaborates with multidisciplinary team members to accomplish patient care goals and objectives of the organization in accordance with established standards, policies and procedures.
- Documents relevant interventions and findings in the patients’ medical records in a timely and accurate manner. Reports critical information to team members on an ongoing basis.
- Acts as an advocate for patient’s health care needs to outside agencies. Coordinates access to Durable Medical Equipment (DME) depending upon insurance availability.
- Completes reports in a timely manner and presents data to the supervisor and the Quality team before the monthly quality meeting.
- Assist with the ACRS/ADT (Active Care Relationship Service/Admission Discharge Transfer) reports and communicates with patients following discharge from the Emergency Department or hospital, ensuring follow up with primary care provider if appropriate.
- Assists with Social Determinants of Health needs during office visits and provides appropriate resources to patients. Makes appropriate referrals to multidisciplinary team and outside agencies as appropriate.
- Responds to in-person, electronic, and telephone requests from providers, patients, and clinical staff.
- Regular and prompt attendance at work.
- All other duties assigned.
Qualifications

Education and Licensure
- High school diploma or GED (Required).
- Licensed Practical Nurse (LPN) or Registered or Certified through the AAMA or AMT as a Medical Assistant (Required).
- Computer and Medical Terminology (minimum 1 year required).
- CPR/BLS certification required within 6 months of employment.
- Experience working as a medical assistant in a clinic (minimum 6 months required).
