MINIMUM QUALIFICATIONS

Education –Preference: Graduate from an accredited professional school of Nursing, BSN RN preferred.

Experience – Three (3) years relevant experience is preferred.

Licenses & Certification – Must possess current State of Oklahoma Registered Nursing License. Must be a Certified Diabetes Care & Education Specialist (CDCES) upon hire or obtain certification within 3 years to maintain job position.

Knowledge & Skills –

  • High degree of knowledge in health care delivery and quality improvement
  • High degree of knowledge and skills in providing preventive and treatment interventions to persons with diabetes in outpatient settings including initial, routine, urgent, annual, follow-up and complication care.
  • Skill in dealing analytically and systematically with problems of organization, work flow, data systems and planning
  • Knowledge of ADA, IHS, MCNDH and Diabetes Program guidelines, regulations, policies and procedures.
  • Ability to provide case management to patients with diabetes using current diabetes care standards and team-derived clinical and educational guidelines
  • Ability to provide diabetes education effectively to groups and individuals
  • Ability to coordinate, plan and schedule program activities within the Clinic
  • Ability to communicate courteously and effectively with patients and their families, medical and administrative staff, health care professionals and the general public both orally and in writing
  • Ability to utilize a personal computer in a Windows environment
  • Ability to maintain a confident professional demeanor

JOB SUMMARY

The purpose of this position is to provide diabetes self-management education and to organize, coordinate, and implement preventive services throughout the MCNDH clinics in an integrative team-care approach with providers. The incumbent will work with team members of the Diabetes Prevention and Management Program and other MCNDH clinic staff to accomplish these services.

WORK ENVIRONMENT

Work is performed in an office and clinical environment. To a lesser extent, work may be performed in community settings, like schools, churches and camps. There is regular and recurring risk of exposure to infectious communicable diseases, potentially hazardous chemicals and bio-hazardous materials. Incumbent must be aware of all risks involved and be willing to take necessary precautions. Some travel may be required.

PHYSICAL DEMANDS

The work requires the demands of normal office and clinical work within the MCNDH clinics. Driving a GSA vehicle may be required to travel to other MCNDH facilities or community events. Must have valid license and be insurable.

ESSENTIAL FUNCTIONS

Satisfactory job performance will be determined by successful execution of the following:

  • Provides quality care related to diabetes case management and complications risk reduction, provides supportive care of diabetes complications.
  • Gathers broad input into development, planning, conducting and evaluating case management services.
  • Coordinates and participates in the provision of diabetes education in outpatient and telehealth settings within the diabetes clinic and walk-in clinic according to the guidelines and resources provided through Local Health Center and the MCNDH Diabetes Program.
  • Documentation of patient care and education to be completed in the EHR per policy guidelines.
  • Develops or adapts curricula for brief teaching of common topics in diabetes self-management education to meet learning needs of persons with diabetes in conjunction with provider encounters.
  • Provides self-management instruction for diabetes medications including insulin administration and adjustment per providers’ orders and guidelines
  • Previews charts for annual monitoring clinic and manages data collection and reporting of yearly clinic results in the medical record and patient report.
  • Tracks timely monitoring and recommended treatment for persons with progressive stages of diabetes complications and communicates and facilitates care needs
  • Assesses current foot care needs and facilitates appropriate referrals.
  • Maintains available or attainable resources as indicated by program evaluation or daily observations
  • Ability to communicate courteously and effectively with patients and their families, medical and administrative staff, health care professionals and the general public both orally and in writing.
  • Ability to utilize a personal computer in a Windows environment
  • Maintains professional licensure/certifications
  • Regular attendance is required.
  • Performs other duties as required
