MINIMUM QUALIFICATIONS

Education – Completion of a Program accredited by the Accreditation Council for Nutrition and Dietetics.

Experience – One (1) year related experience. Three (3) years’ relevant experience is preferred.

Licenses & Certification – Incumbent must be registered (RD) with the Commission on Dietetic Registration and licensed by the State of Oklahoma as a Dietitian (RD/LD). If incumbent has current provisional licensure, the licensure will be required within 6 months of employment. Certification as a Certified Diabetes Care & Education Specialist (CDCES) preferred or must obtain certification within 3 years to maintain job position.

Knowledge & Skills –

  • Knowledge of diabetes and its impact upon the Native American population.

  • Knowledge of nutrition as it relates to diabetes, diabetes awareness and prevention in communities.

  • Knowledge and skill in nutrition counseling and education in a clinical setting.

  • High degree of knowledge in health care delivery and quality improvement.

  • High degree of knowledge and skills in providing preventive and treatment interventions to persons with diabetes in outpatient settings including initial, routine, urgent, annual, follow-up, and complication care.

  • Knowledge of ADCES, ADA, IHS, MCNDH and Diabetes Program guidelines, regulations, policies and procedures.

  • Ability to provide case management to patients with diabetes using current diabetes care standards and team-derived clinical and educational guidelines.

  • Ability to assess patients’ nutritional needs and prepare nutritional care plan.

  • Ability to effectively plan, coordinate diabetes activities and nutrition-related education to groups and individuals within the clinic and community setting.

  • High degree of knowledge in diabetes self-management education and standards required by the American Diabetes Association’s Recognition program.

  • Knowledge of cultural competencies in the Native American Indians relevant to diabetes and its prevention and care.

  • Ability to communicate courteously and effectively with patients and their families, medical and administrative staff, health care professionals and the public, both orally and in writing.

  • Ability to utilize a personal computer in a Windows environment.

  • Ability to maintain a confident professional demeanor.

JOB SUMMARY

The purpose of this position is to provide nutrition counseling, diabetes self-management education and supplemental diabetes care that supports the Local Clinic staff in providing initial, routine, urgent, annual, follow-up, and specialty care to persons with diabetes or at risk for diabetes. The incumbent will work with both the staff of the Diabetes Prevention and Management Program and the Local Clinic staff to accomplish job tasks.

WORK ENVIRONMENT

Work is performed in an office and clinical environment with normal risk of exposure to the hazards associated with each setting. To a lesser extent, work may be performed in community settings, like schools, churches, and camps. There is regular and recurring risk of exposure to infectious communicable diseases, potentially hazardous chemicals and bio-hazardous materials. Incumbent must be aware of all risks involved and be willing to take necessary precautions. Some travel may be required.

PHYSICAL DEMANDS

Employee is regularly required to sit, stand, walk, bend, stoop, stretch and lift equipment or teaching aides while performing duties. Driving a GSA vehicle may be required to travel to other MCNDH facilities. Must have a valid driver’s license and be insurable.

ESSENTIAL FUNCTIONS

Satisfactory job performance will be determined by successful execution of the following:

  • Provides comprehensive Medical Nutrition Therapy (MNT) for persons at risk for or with newly diagnosed diabetes, persons with established disease to improve control and reduce risk factors and persons who are progressing through various stages of diabetes complications.

  • Provides quality care related to nutrition services using evidence-based guidelines for nutrition.

  • Helps in planning, developing, implementing curricula to meet the needs of the patient population, conducting evaluating nutrition and diabetes self-management services at local clinic.

  • Coordinates & participates in provision of diabetes education in outpatient & telehealth settings within the Local Clinic, Diabetes Program, & various community settings according to guidelines and resources provided via the Local Indian Health Clinic and the MCNDH Diabetes Program.

  • Participates in identifying local clinic program resources and activities including those in the clinic and area communities to meet patient’s health needs.

  • Coordinates the local clinics evaluation of the objectives of nutrition services stated in the annual program plan, IHS Diabetes Audit, Patient satisfaction surveys, Community diabetes awareness surveys, diabetes self-management program and Community prevention program.

  • Provides self-management instruction for diabetes medications including insulin administration and adjustment per provider orders and guidelines.

  • Monitors and facilitates each patient’s progress & completion of program requirements, conducts follow-ups as needed to ensure completion of routine care, annual care, specialty care and program requirements.

  • Documents assessment data, education provided, clinical interventions and plans including recommendations and goals for each participant in the electronic medical record.

  • Utilizes a team approach to provide services, collaborates and communicates with team. Serves as a resource to other team members, assists with education events/focus sessions.

  • Interacts with interdisciplinary team of health professionals.

  • Contributes to and participates in a continuous quality improvement process.

  • Participates in formal cultural training and events initially and annually thereafter.

  • Maintains professional licensure/certifications.

  • Regular attendance is required.

  • Performs other duties as required.
