Muncie Primary Care is seeking a motivated, compassionate, and highly organized Registered Nurse (RN) to join our Chronic Care Management team.

Unlike traditional bedside nursing, this position focuses on developing ongoing relationships with patients managing chronic conditions. You'll use your clinical judgment to identify concerns, coordinate care, educate patients, and help prevent complications and unnecessary hospitalizations.

You'll have the autonomy to manage your own patient panel while collaborating closely with physicians, nurse practitioners, and other members of our care team.

Responsibilities

• Independently manage a panel of chronic care patients, prioritizing outreach, follow-ups, and care coordination.
• Build lasting patient relationships through proactive communication and personalized education.
• Assess patient needs, recognize changes in condition, and coordinate timely interventions.
• Support patients with medication adherence, chronic disease management, preventive care, and lifestyle changes.
• Collaborate with physicians, nurse practitioners, and nursing staff to ensure coordinated patient care.
• Maintain accurate, timely documentation in our electronic health record (EHR) and ChronicCareIQ platform.
• Meet established performance goals related to patient engagement, documentation, and care management.

Qualifications

Required:

• Active, unrestricted Indiana Registered Nurse (RN) license.
• Strong clinical assessment, communication, and documentation skills.
• Ability to independently organize and manage patient care responsibilities.
• Comfortable providing primarily telephone-based patient care, education, and follow-up.

Preferred:

• Experience in primary care, outpatient nursing, chronic disease management, care coordination, or case management.
• Experience using electronic health records (EHR) or digital care management platforms.

Prior chronic care management experience is a plus, but not required. We are willing to train the right RN who brings strong clinical skills, initiative, and a commitment to excellent patient care.
