Now Hiring: Intake Coordinator – Home Health
Contract: 13 Weeks
Shift: Day Shift, Monday–Friday, 8:00 AM–4:30 PM
Schedule: 5 × 8-hour shifts
Pay Rate: $35-$40/hour
Requirements:
- Current RN, LVN, or LPN license in the state of practice
- CPR certification required
- Minimum 1.5 years of experience
- Epic experience required
- Healthcare experience in a clinic, home health, SNF, or hospital preferred
- Associate/technical degree in nursing or equivalent education and experience required
Job Responsibilities:
- Process home health referrals and coordinate patient admissions.
- Communicate with clinical teams, physicians, and referral sources.
- Review patient needs and coordinate appropriate care.
- Verify insurance and explain benefits.
- Maintain accurate documentation and HIPAA compliance.
