CLINICAL SOCIAL WORKER 

JOB SUMMARY

• The Clinical Social Worker provides comprehensive social work services for the patient population served.

ESSENTIAL FUNCTIONS OF POSITION

• Psychosocial assessment of families
• Participation in the development of an interdisciplinary treatment plan
• Provides educational, supportive and counseling services to families
• Coordination of services with community agencies
• Work with the interdisciplinary team to address barriers to safe and timely discharge. 
• Achieve high quality and cost effective patient outcomes across the continuum of care
• Functions with a focus on patient safety, customer service, and family centered care
• Participates in performance improvement initiatives for the department

POSITION REQUIREMENTS

Educational Requirements

• Masters Degree in Social Work from a University accredited by the Council on Social Work Education.

Licensure, Certification and Registration

• Current license to practice Social Work in the State of Maryland. 

Work Orientation and Experience

• Prior experience as a Social Worker in an acute or rehabilitation hospital with pediatric experience preferred. Experience in child welfare, family service, or mental health setting, is also desirable. Experience with organization and coordination of care for complex families; discharge planning and interagency coordination.

Knowledge, Skills and Abilities

• Knowledge of normal growth and development for infants to young adults preferred
• Knowledge of family functioning and the nature of chronic illness/disability and its impact on the patient and family preferred
• Skill in individual, family and group treatment modalities preferred
• Ability to effectively communicate both orally and in writing with interdisciplinary team members, family members, community agencies, referral sources and payers.
• Ability to work effectively with an economically and culturally diverse population.
• Ability to balance patient/family needs with constraints imposed by third-party payers and agency regulations.
• Knowledge of third party payer systems including Medicaid and Managed Care Organizations

• Time management and organization skills
• Ability to effectively negotiate and build strong collaborative relationships

REPORTING RELATIONSHIPS

Supervision Received

• Reports to Social Work Team Leader and Director of Care Management/Social Work/Admissions/Language Services. Individual and group supervision available for LMSW licensees. 

• Supervision Exercised

• LSCW-C with 3 years experience at MWPH, may provide clinical supervision to Social Work Students observational experiences.

TOOLS/EQUIPMENT USED

• Computer, telephone, fax machine, photocopier, on-line information system, personal pager

PHYSICAL ENVIRONMENT/WORKING CONDITIONS

• Frequent stress due to multiple demands, shifting priorities, shifting populations and tight time frames. Risk of exposure to body fluids, infectious waste or blood borne disease. Employees are expected to exercise good judgment in the use of personal protective equipment and adherence to established protocol.
