Minimum Qualifications:
- RN license required
- Leadership
- Tactful
- Knowledge of nursing and medical practices and procedures
- Care Manager experience is preferred
Job Duties:
- Facilitate patient care by assessing enhanced care - long-term behavioral health patient needs, evaluating treatment options, creating treatment plans, coordinating care, and gauging progress.
- Facilitate and actively participate in interdisciplinary planning of clinical care of patients and assist with outcome report completion.
- Serve as care liaison between patients, physicians, care team members, and discharge planner by coordinating, monitoring, and communicating patients’ progress.
- Initiate and implement care plans as necessary through monitoring and re-evaluation to accommodate changes in treatment or progress.
- Communicate discharge information to physicians and payers with outcome data, as appropriate.
- Make written and oral reports/recommendations concerning the activities of your shift as required.
- Ensure that all nursing service personnel comply with their respective job descriptions.
- Participate in the development, maintenance, and implementation of the facility’s quality assurance program for the nursing service department.
- Participate in facility surveys (inspections) made by authorized government agencies as may be requested.
- Conduct accident/incident investigations and give completed reports to the Director of Nursing.
- Perform administrative duties such as completing medical forms, reports, evaluations, studies, and charting
