What are the responsibilities and job description for the Nurse III (Restorative Nurse) position at MS State Veterans Home-Tradition?
RESTORATIVE NURSE JOB DESCRIPTION
(duties include, but are not limited to)
· Obtain weekly and monthly weights and record them in the system
· Managing interventions for weight loss
· Manage restorative dining program and monitoring eating habits on all hall and communicating with CNA’s about how residents are eating
· Work closely with Dietician about weight loss and interventions (supplements and diet change orders)
· Call the RP’s about falls and weight loss and document in the nurse notes
· Conduct IDT meeting on assigned day and document in nurse notes
· Communicating weight loss and overall decline and falls to MD
· Completing the user defined assessments: Side Rail, Dehydration, Restrains, Hot Beverage Assessment and Smoking Assessments
· Checking the appropriateness of intervention for Incidents and Accidents
· Obtaining Therapy recommendations for residents that are on restorative caseload
· Complete monthly Restorative Report and give to Administrator and DON
· Dining room duty as assigned
· Participate in ON Call Rotation
· Working medication cart when staff on halls are short
· Assessing all new admissions for ADL needs
· Oversee restorative CNAs
· Manage the Bowel and Bladder training program
· Manage the falling leaf program for residents with repeat falls
· Managing the investigation process for falls and Incident/Accident Reports
This includes: making sure the I/A report is filled out correctly and closed in the system, Make sure there is a post incident action (intervention), Make sure there are witness statements, Make sure RP was notified, Make sure the fall assessment was done
