What are the responsibilities and job description for the Nurse III Wound Care position at MS State Veterans Home-Tradition?
WOUND CARE NURSE DUTIES
(duties include, but are not limited to)
· Perform daily wound treatments (specifically decubitus, wound vac, etc.)
· Measure wounds/decubitus and document weekly
· Report, refer declining or non-healing wounds (to MD, wound clinic or wound consultant)
· Obtain C/S or change in treatment as indicated
· Complete Braden scale in Skin Wizard in Wound Assessment Manager with the wound measurements and assessments
· Assess new skin issues with documentation
· Provide IDT members (Administrators, DON, ADON, Restorative Nurse, Nurse Managers/RN Supervisors, Dietitian) with printed copy of Wound and Skin status report in Wound Management Manager weekly
· Weekly risk management meetings with documentation
· Perform monthly nail care on Diabetics and residents on Coumadin
· Provide monthly QA wound report
· Complete Pressure Ulcer Risk Evaluation in User Defined Assessments in Quality Assurance with MDS assessments (Annual, Quarterly, Significant change), Admissions and Hospital returns (Re-admissions)
· See all Admissions and Hospital returns/Re-admissions (preferably with the Unit Manager/RN Supervisor) for a thorough skin assessment, document skin status and all wounds/skin issues
· Follow Infection control practices
· Keep Wound treatment cart clean, maintain adequate wound supplies for the residents
· If other Nurses are assisting with documentation, the Wound Nurse is overall still responsible for completion
· On call rotation
· Work a medication cart as needed
