Job Title: Outpatient Therapy Clinical Coordinator
Department: Outpatient Therapy
Supervisor: V.P. Community Services and Business Development
Job Objective:
This position provides clinical and operational leadership for Mozaic Senior Life's outpatient rehabilitation program while maintaining an active patient treatment caseload. Approximately 75–80% of time to direct patient care and 20–25% to coordination and leadership responsibilities. This a key role responsible for supporting high-quality, patient-centered therapy services that support wellness, recovery, functional independence, and aging in place as well as daily clinic operations, operational oversight staff coordination, program growth, and community engagement.
Minimum Qualifications:
Education: Bachelor's degree required; advanced degree preferred
Experience: Minimum of 3–5 years of outpatient therapy and supervisory experience in geriatrics or neurological rehabilitation setting
Licensure: Licensed (unrestricted) Physical Therapist (PT) in the State of Connecticut

Essential Job Functions:
Clinical Leadership
- Maintain an active outpatient therapy caseload
- Lead by example through clinical excellence and patient engagement
- Support evidence-based treatment approaches and quality outcomes
- Promote interdisciplinary collaboration across therapy disciplines
Operational Oversight
- Assist with therapist scheduling, workflow, and clinic operations
- Monitor cancellations, no-shows, patient access, and scheduling efficiency
- Support productivity and patient satisfaction initiatives
- Assist with operational problem-solving and workflow improvements
Program Growth & Innovation
- Assist in developing new therapy and wellness programs
- Support referral development and community outreach initiatives
- Identify opportunities to expand services aligned with community needs
- Collaborate with leadership on strategic growth initiatives
Community Engagement & Partnerships
- Build positive relationships with physicians, referral sources, and community partners
- Collaborate with internal Mozaic programs to support integrated care pathways
- Represent outpatient therapy services professionally within the community
Regulatory & Quality Oversight
- Ensure compliance with federal, state, payer, and organizational requirements
- Support audit readiness and documentation compliance
- Participate in quality improvement and patient safety initiatives
Team Leadership & Development
- Provide day-to-day support and guidance to therapy staff
- Foster a culture of accountability, teamwork, and innovation
- Support onboarding, mentoring, and staff development efforts
Other related duties as assigned.
Physical and Mental Requirements:
- Ability to sit or stand for long periods of time (often 6-8 hours per day).
- Ability to lift, transfer, and reposition patients (typically up to 50 lbs. independently; more with assistance or equipment)
- Frequent walking, bending, stooping, kneeling, crouching, and reaching.
- Ability to perform manual therapy techniques requiring upper body strength and coordination as well as hand/eye coordination such as keyboard skills, writing or picking up paper.
- Ability to communicate clearly and compassionately while maintaining professionalism and empathy in emotionally demanding situations.
- Ability to motivate people who may be reluctant, fatigued, declining or who have cognitive issues.
- Ability to maintain focus with some interruptions while supervising clients and preventing falls and injuries during mobility tasks.
- Able to hear sounds within some background of noise as in answering the phone, etc.
- Visual acuity to see clients, a computer and read printed documents.
- Ability to use company software and equipment to accomplish electronic tasks.
- Ability to stay focused on tasks for more than 60 minutes at a time.
- Ability to understand and relate to the reasons behind several related concepts.
- Ability to remember multiple tasks /assignments given to self and others over long periods of time.
Environmental Conditions:
- Rare exposure to blood, body tissue or fluids.
- Possible exposure to high/low temperatures.
