Your Role as a Physical Therapist:
- Evaluate, plan, and implement personalized therapy for residents with temporary or long-term physical conditions.
- Provide treatments in line with the plan of care and modify as needed based on resident progress.
- Educate and support patients, families, and caregivers throughout the rehab journey.
- Monitor and document patient responses, progress, and outcomes using the Net Health electronic documentation system (preferred).
- Collaborate with our multidisciplinary team to promote residents' healing, independence, and overall quality of life.
What We're Looking For:
- Current Physical Therapist license in the State of Pennsylvania (in good standing).
- SNF/Long-Term Care experience preferred.
- Strong communication and interpersonal skills.
- Familiarity with modern PT practices and documentation systems.
- Compassionate, patient-centered mindset.
