Nurse (RN or LPN) – Long-Term Care / Skilled Nursing
Shifts: 7:00 pm – 7:00 am

We are seeking a compassionate and dependable Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our Long-Term Care / Skilled Nursing team. In this role, you will provide high-quality nursing care to residents with chronic conditions and complex medical needs while helping ensure their comfort, safety, and well-being overnight.

You will serve as a key clinical presence during the night, monitoring resident conditions, responding to changes in health status, and collaborating with the care team to provide excellent resident-centered care.

Responsibilities

• Perform resident assessments, monitor vital signs, and document care in PCC.
• Administer medications, treatments, and injections following facility protocols.
• Provide skilled nursing care for residents with chronic illnesses, post-acute recovery needs, and mobility limitations.
• Monitor and manage clinical needs such as wound care, IV therapy, catheter care, G-tubes, and ostomy care.
• Observe for changes in resident condition and respond appropriately.
• Respond to medical events including falls or acute health concerns.
• Maintain accurate documentation and communicate important updates to the oncoming shift.
• Follow infection control, safety, and regulatory standards.

Qualifications

• Current RN or LPN license in good standing.
• Experience in long-term care, skilled nursing, or rehabilitation preferred.
• Experience with wound care, tube feedings, and chronic disease management preferred.
• Ventilator or tracheostomy experience is a plus but not required.
• Strong clinical judgment and ability to work independently on night shift.
• Compassionate, resident-focused approach to care.

Pay: $30.00 - $45.00 per hour
