JOB SUMMARY:
The Sponsored Residential Services Facilitator Supervisor assists in the oversight of day-to-day activities of Sponsored Residential Homes including but not limited to providing direct and indirect periodic service consisting of assessment, Person-Centered Planning and monitoring/follow up on behalf of individuals served and their respective provider of Sponsored Residential Services. Activities also include but are not limited to documentation, participation in regularly scheduled meetings and monitoring services to comply with licensing regulations and in accordance with MRCSB policies and procedures.
The Sponsored Residential Services Facilitator Supervisor duties are to guide the family/individual receiving services in employing, training, and supervising attendants as needed in the following entitlements: ID/DD Waiver assesses an individual’s level of need and identifies the appropriate CD service; with input from the individual the Services Facilitator develops the Plan of Care (POC) and provides instruction and development for the Employee Management Manual. The Sponsored Residential Services Facilitator Supervisor provides training to the Individual/Employer of Record (EOR) around the role and responsibilities of the EOR, explains enrollment documentation necessary and required by the fiscal agent and assisting with the completion. Additionally, monitors and regulates service authorizations to ensure reimbursement to Consumer Directed (CD) staff by acting as a liaison between staff, employer, and other agencies including fiscal agent, attends ongoing meetings and trainings, works with other providers/agencies and Support Coordinator as part of Individual’s treatment team and provides ongoing support to the CD EOR.
ESSENTIAL FUNCTIONS:
- Monitor strict adherence/compliance with VA Licensure, Medicaid and Human Rights Standards by following all protocols and procedures ensuring monthly, quarterly, scheduled, unannounced, and annual recertification visits/contact in SRS provider homes, as well as maintaining ongoing compliance with policies/procedures, accordingly.
- Responsible for reviewing, assessing and summarizing served individual needs utilizing reports, electronic information (via electronic medical record), observation, and other sources in formulation of monthly summaries, quarterly reviews, Individual Service Plans and other duties to ensure continuation of appropriate placement of served individuals.
- Monitor service delivery to assure quality of care as well as continued appropriateness of service via dialoging, observing, providing support/education, reporting and aiding SRS providers in overcoming deficiencies, and overall regulatory compliance of Sponsored Residential Providers.
- Participate in activities in accordance with the implementation of person-centered practices by ensuring proper care and treatment, preventing duplication of services, and by working with support coordinators as well as other relevant providers for ongoing service delivery.
- Responsible for coordinating treatment and linking consumers and providers with resources as needed, as well as coordinate and promote the use of natural supports in planning services/supports with the individual/guardian/legally responsible person and the treatment
- Assist in the provision of 24 hour on-call services for emergencies and Residential Provider consultation.
- Ensure accurate billing is completed and submitted electronically in a timely fashion, reviewing, providing quality assurance and approving/sending back for correction all service notes per caseload each month.
OTHER DUTIES:
- Attend regular group or individual supervision as required.
- Participate in ongoing Quality Improvement activities including Peer Review.
- Provide high quality customer service, serving as a positive representative for the agency.
- Perform other duties as assigned by the supervisor.
QUALIFICATIONS:
- Knowledge of intellectual and developmental disabilities and related conditions.
- Good oral and written communications.
- Ability to compile data reflective of assessments, goals and objectives.
- Ability to interact with people in a supportive and therapeutic manner.
- Willing to work flexible hours.
- Ability to work as part of a team.
- Knowledge of supervisory principles.
- Skills in data collection, needs assessment, and person-centered treatment planning.
- Ability to work independently and with supervision available at broadly spaced intervals.
- Criminal background and Department of Social Services registry check required.
The Sponsored Residential Services Facilitator Supervisor and all Consumer-Directed Services Facilitators shall complete required training and competency assessments (and achieve a score of at least 80% on each of the individual modules). Computer based training is available at: http://www.vcu.edu/partnership/servicesfacilitators/index.html Satisfactory competency assessments results (80% on each of the individual modules) shall be maintained in the Services Facilitator’s personnel record and are subject to review by DMAS staff and contractors.
Job Requirements
EXPERIENCE/EDUCATION REQUIRED:
A minimum of an associate’s degree from an accredited college in a health or human services field or be a registered nurse currently licensed to practice in the Commonwealth, and possess a minimum of two years of satisfactory direct care experience in supporting individuals with disabilities or older adults; or
A bachelor's degree or higher in a non-health and human services field with a minimum of three years satisfactory direct care experience in supporting individuals with disabilities or older adults.
