Schedule: Full-Time
Mount Carmel Nursing & Rehabilitation is seeking a dedicated Social Services Director to lead the Social Services Department within our nursing home. This role serves as a liaison and advocate for residents’ social interests and is responsible for planning, organizing, implementing, evaluating, and directing the overall operations of the Social Services Department.
Position Overview
The Social Services Director ensures that residents’ medically related social and emotional needs are identified and addressed through comprehensive assessments, individualized care planning, and ongoing support. This position plays a vital role in resident adjustment, family communication, crisis intervention, and regulatory compliance.
Key Responsibilities
Develop comprehensive social assessments and individualized care plans that identify medically related social and emotional needs, establish realistic goals, and outline specific interventions
Actively participate in pre-admission and admission processes
Serve as a compassionate resource to residents and family members during periods of adjustment, crisis, or special need
Document residents’ behavioral and psychosocial status accurately and timely
Provide individual and group support services to residents and families
Establish, coordinate, and support a volunteer program designed to meet specific social service needs
Orient residents and families to the nursing center and assist with transition and adjustment to the new environment
Ensure compliance with federal, state, and local regulations governing social services in long-term care settings
Qualifications
Minimum requirement: Bachelor’s Degree in Social Work or a related field
Previous long-term care (LTC) experience desirable
Past supervisory experience preferred
Strong knowledge of social service practices, procedures, laws, regulations, and guidelines governing long-term care
Excellent communication, organizational, and interpersonal skills
Ability to work independently while managing multiple priorities effectively
