SUMMARY
Provide personal care under the supervision of a nurse according to the patient's plan of care.
Collaborate with transdisciplinary team in a variety of settings.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Provide personal care and assist with ADL's according to the Plan of Care - POC
- Provide safe environment for patients
- Safely ambulate/transfer patient as appropriate
- Assist with passive and active ROM exercises as directed by the nurse
- Prepare nutritional snacks as appropriate for the patient
- Under the direction of the nurse, perform simple wound care, skin care, and bowel care
protocols according to the plan of care where appropriate - Recognize and promptly report any changes in the mental or physical status of the
patient to the nurse - Record in clinical record, on appropriate documents, the activities provided and
observations of patient at the time of visit - Complete all charting, timesheets and mileage logs as defined by the agency policy and
procedures - Attend and actively contribute during patient care conference
- Participate in HNA discipline meetings, team meetings, staff meetings, retreats and in-services as appropriate
- Attend in-services to obtain 3 education hours per quarter.(need 12 a year)
- Participate in the on-call rotation as scheduled (Home Care only)
- Assist the Team Leader with other designated activities as necessary (scheduling, supply
organization, answering office telephone, etc.) - Demonstrate positive customer-service responsiveness according to agency standards
- Flexible in meeting patient/family needs that require variable shifts and weekends
- Participate in chart review quality improvement activities and other meetings as
assigned - Participate in coordination of and communication with volunteer staff
- Provide basic caregiving education to caregiver/family members, such as turning and
positioning, transfer techniques, and changing linens - Complete annual competency requirements
QUALIFICATIONS
(EDUCATION and/or EXPERIENCE)
- Must be state tested nursing assistant in good standing and have 12 months of experience
as a nursing assistant
CERTIFICATES AND LICENSES
- Must be a Florida Certified Nursing Aide-CNA
