About the Role:
As a Licensed Practical Nurse (LPN) with Morristown Post Acute Rehabilitation & Nursing, you will be responsible for:
- Providing direct patient care, including administering medications, wound care, and other treatments as ordered by physicians
- Developing and implementing individualized care plans in collaboration with the interdisciplinary team
- Monitoring and reporting patient condition and changes to the healthcare team
- Educating patients and families on disease management, treatment options, and post-discharge care
