Responsibilities:
Provide direct patient care to residents, including administering medications, documenting patient information, and conducting routine vital sign checks
Develop and implement individualized care plans in collaboration with the interdisciplinary team
Assist with daily living activities, such as bathing, dressing, and toileting
Monitor and report any changes in patient condition to the nursing team
Collaborate with therapists to develop and implement individualized rehabilitation plans
Maintain accurate and detailed records of patient care and progress
Requirements:
Current LPN license in the state of New Jersey
Ability to work effectively in a fast-paced environment
Compassionate and caring approach to patient care
Strong communication and teamwork skills
