JOB SUMMARY
Morris Heights Health Center (MHHC) is seeking an enthusiastic individual with knowledge of I IIV and Case Management/Navigator to perform intense case management and outreach services for HIV positive patients. The candidate must be willing to perform robust follow up including. but not limited to home visits, patient escorts lo appointments, modified direct observed therapy, referrals, outreach, tabling events, patient recruitment. and health education to list a few. The candidate must be highly motivated, possess strong writing and analytical skills. The Case Manager/Navigator will be responsible for submitting monthly reports. ensure program goals are met, and follow related Program & Organizational policies, procedures, and protocols.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Perform phone and home visit monitoring to ensure viral load suppression and engagement in care.
- Perform treatment adherence support, risk, and harm reduction counseling.
- Develop & maintain a referral network of community- based organizations. SRO's and Social Services organizations that provide a wide variety of services for impacted patients.
- Conduct street outreach, tabling events, in order to recruit patients, from local businesses, schools, hospitals and clinics.
- Provide newly diagnosed and HIC positive patients with the prevention counseling and refer to Positive life workshop.
- Conduct referrals & escort patients to needed services.
- Coordinate with the Data team & Manager to support quality assurance activates.
- Participate in multidisciplinary team conferences, including case presentation.
- Participate in quality improvement activities.
- Maintain required documentation of all services provided in the patients' electronic medical record & Eshare.
- Complete timely reports as directed by the Manager
- Other duties as assigned.
JOB QUALIFICATION
- Associates degree preferred (with experience). Bachelor's degree preferred.
- A minimum of two years of experience working with HIV positive clients preferred.
- Ability to interfere with electronic health record system.
- Strong verbal and written communication.
- Strong knowledge of New York based community resources for Persons living with HIV.
- Proficient in Word and Excel.
- Ability to thrive in a fast-paced environment, think strategically and utilize superior judgment in making decisions.
- Bi-Lingual. Spanish speaking, preferred.
