RN Case Manager - Full Time

We are excited to welcome a dedicated and skilled Registered Nurse to our healthcare team as a Case Manager. This full-time position offers the opportunity to significantly impact patient care and well-being in a collaborative and supportive environment.
Key Responsibilities:
- Deliver exceptional patient care to diverse populations, considering various health conditions and age groups.
- Perform comprehensive assessments and accurately document patient histories.
- Create and implement personalized care plans tailored to individual patient needs.
- Administer prescribed medications and treatments, monitoring patient progress and recording vital signs.
- Work closely with a multidisciplinary team to ensure integrated care and achieve optimal patient outcomes.
- Provide education to patients and families on health management, disease prevention, and available treatment options.
- Maintain meticulous patient records and documentation.
Qualifications:
- With Home health experience as RN
- Knows Oasis but not required
- Graduate of an accredited nursing program; BSN, PHN preferred.
- Current Registered Nurse license for practice in the relevant state(s).
- A minimum of one year of experience in an acute care setting or equivalent experience.
- Valid driver's license with insured and well-maintained vehicle as per state and agency guidelines.
- Current CPR certification.
- Excellent communication and interpersonal skills.
