Patient Educator:

- Develops and implements educational services for a designated patient population to address their specific health care issues
- Assesses the needs of patients, coordinates health educational programs, and prepares educational materials
- Coordinating and supporting mental health care provided in the primary care clinic. In supporting mental health care, the PE will be providing patient education and follow-up focusing on treatment adherence, treatment effectiveness, and treatment side effects
- Coordinating referrals to clinically indicated services and tracking and maintaining measurement data in Montefiore’s electronic medical records and in the Care Management Tracking System (CMTS) registry
- Utilizing evidence-based techniques when indicated such as Motivational Interviewing, and Behavioral Activation.
- Annual assessment
- Fit test
- Bachelor's degree with a major in Social Work, Health Education, or similar fields
- One to three years work experience in related health field
- Experience with screening for common mental health/substance use disorders
- Strong written and verbal communication skills; strong organizational skills
- Ability to work in a fast-paced environment and prioritize activities
- Proficiency with computers and Microsoft Office software applications
- Bilingual (English/Spanish) is preferred
Shift: Monday – Friday 8:30a – 5:00p
Associates are eligible to apply for a lateral transfer after completion of one (1) year of service in their present position, with the following exception: an Associate who has received a written warning is not eligible for a transfer/promotion prior to six (6) months from date of issuance; an Associate who has been suspended is not eligible for transfer/promotion prior to one (1) year from date of suspension. Part-time Associates who have completed their probationary period can move to full-time vacancies within their department provided such movement will not unreasonably reduce the operational efficiency of that department. Probationary period is three (3) months for full-time employees, and four (4) months for part-time employees who works more than 3/5 of the normal workweek.
