Associates are eligible to apply for a transfer after completion of one (1) year of service in their present position, with the following exception: an Associate who has received a written warning is not eligible for a transfer/promotion prior to six (6) months from date of issuance; an Associate who has been suspended is not eligible for transfer/promotion prior to one (1) year from date of suspension. Probationary period is four (4) months for full-time employees, and six (6) months for part-time employees.
Overview:
The Home Care Liaison facilities referrals to home care and/or hospice by gathering and interpreting medical and financial data, meeting with patients and families, and working with case managers to ensure optimal transition from hospital or other facilities to home.
Job Responsibilities:
- Makes initial contact for clinical, social, psychological and financial assessment of home care and/or hospice services
- Provide accurate and thorough documentation either via phone, fax, or computer to agency for additional data entry
- Assists with patient's insurance to determine home health and/or hospice coverage
- Interprets home care and hospice policies and acts as a resource for case managers, physicians, hospital and community for effective public relations and marketing of services
Education & Experience Qualifications:
- BSN ( Preferred)
- Licensed & Registered RN nurse
- 2 years of Previous experience in home health, hospice, or healthcare coordination is preferred.
