Candidate must be willing to travel throughout Cattaraugus County on a regular basis.
Grade 307: This is a full time position, working from home, but does require significant travel through Cattaraugus County.
The minimum and maximum annual salary that Monroe Plan believes in good faith to be accurate for this position at the time of this posting are $60,949 - $78,362. POSITION SUMMARY
The Team Lead is responsible for providing coaching, training, and administrative support oversight for the care management staff. The Team Lead is accountable for staff development and performance improvement and for promoting appropriate and effective care through established monitoring systems and professional staff growth. The Team Lead conducts weekly team meetings, manages relationships with hospitals, runs regular case presentations, ensures continuity between fieldwork and meetings, reviews the entire caseload, assists in covering caseloads when needed, promotes model fidelity, staff productivity and staff wellness, monitors quality of documentation, updates resource lists, monitors phase dates, assists in covering caseloads when needed, and manages the caseloads for all staff. This position will monitor and address problems that occur on a day-to-day basis for operational concerns. The Team Lead ensures that the treatment provided is consistent with Monroe Plan philosophy and meets the current standards for quality of care and other requirements as defined by NYS Department of Health (DOH) and Office of Mental Health (OMH). The Team Lead role will require significant multi-tasking in a fast paced, challenging environment. The role will also require field-based work, periodic on-call coverage, and a willingness to work flexible hours.

OTHER FUNCTIONS AND RESPONSIBILITIES
- Demonstrate cultural awareness, cultural knowledge, and cultural skills in all aspects of job role.
- Attend and participate in team meetings and organizational/departmental training.
- Performs other duties as assigned.
Position Limitations:
- Cannot perform any tasks which are governed by license or registration (i.e. cannot answer questions or make recommendations RE diagnosis, medications or treatment).
- Cannot transport active Monroe Plan members at any time.
- Cannot perform hands on care.
MINIMUM REQUIREMENTS/LICENSES/CERTIFICATIONS
- Licensed Practitioner of Healing Arts (LPHA) – RN, LMSW, LCSW, or Mental Health Counselor.
- Bachelor’s Degree with 3-5 years post-licensure experience working with individuals who have mental health, or co-occurring mental health and substance use disorders.
· Minimum of 5 years’ experience in case management or a clinically related area.
- 2-3 years' previous leadership experience with strong team building and mentoring skills.
- Knowledge of counseling principles and methods for mental illness and substance use disorders.
- Knowledge of treatment, rehabilitation, and community support programs as they relate to members, members’ families, and staff.
- Knowledge of techniques for identifying, assessing, and preventing potentially violent behavior, including crisis management and de-escalation techniques.
- Ability to develop, evaluate, implement, and modify treatment interventions to meet the needs of individual recipients.
- Strong leadership and organizational skills required.
- Excellent planning, organization, and implementation skills required.
- Must demonstrate excellent oral/written communication skills.
- Must possess excellent computer skills and have mastery of clinical software required for SOS program documentation and administration.

- Candidates will need a NYS driver’s license and to own or have access to reliable transportation that enables them to fulfill travel requirements of the job.
