The Health Home Care Manager provides care management services to specific populations eligible for Health Home services. Provides information, referrals, consultation, and/or care management on health and psychosocial issues. This position works with substantial independence in the field, with consultation available from the Supervisor, as needed.
Candidates must live in the Tompkins County, NY area or similar vicinity as the position does require home visits with our members. Candidates must possess at a minimum, a Bachelor's Degree in Social Work, Psychology, Sociology, Human Services or other applicable field of study and have at least 2 years of experience providing direct services to people with Serious Mental Illness, developmental disability, alcoholism or substance abuse and/or children with SED; or linking individuals with these diagnosis to a broad range of services essential to successful living in a community setting.
The minimum and maximum annual salary that Monroe Plan believes in good faith to be accurate for this position at the time of this posting are $46,948 - $57,380.
