Forensic Psychiatric Nurse Practitioner

  • Part TimeMake a difference and work for an agency where you matter!The Forensic Assertive Community Treatment (FACT) Team Psychiatric Nurse Practitioner is a member of thecommunity based multidisciplinary treatment team, and in conjunction with the Clinical Director of ForensicAssertive Community Treatment Team provides oversight of the delivery of clinical services.

The FACT TeamNurse Practitioner has three main functions: 1) Provision of psychiatric assessment and treatment utilizing anindividual centered approach while taking into consideration complex medical issues; 2) sharing knowledge ofpsychiatric illness and treatment, with the goal of educating and guiding the multidisciplinary team; and 3)oversight of the development, maintenance, and supervision of medication administration. Duties include:

  • Upon admission, conduct person-centered psychiatric assessments.
  • Collaborate with nursing staff regarding individual medical needs and recommendations for referrals to community physicians for further assessment and treatment.

Collaborate with medical providers as needed.

  • Meet with individuals at the office and in their homes to prescribe psychiatric medications, provide medication education, assess efficacy of medications, and monitor for side effects.

Conduct regular assessments of therapeutic responses and side effects and educate individuals regarding psychotropic medications.

  • With other team members, provide support, education, and counseling to family members of individuals to help them understand mental illness and how to best support the recovery of the individual, as needed and if indicated.
  • Actively collaborate with inpatient providers during the acute or long-term hospitalization of an individual.
  • Participate in and supervise the development of the comprehensive treatment plan for each patient at treatment planning meetings.
  • Direct and collaborate with the registered nurses to develop, revise, maintain, and supervise team pharmacological and medical treatment as well as medications policies and procedures.
  • Provide transportation to individuals when necessary.

In addition, an employee must be approved to drive agency and personal vehicles for business purposes by maintaining a valid NYS driver’s license and must be approved by the agency’s vehicle insurance carrier.

An employee must always drive in a safe manner and use all safety devices.

  • Mohawk’s programs and offices are multi-level with stairs only.

Individual services in the community include visits to individuals’ apartments which can be on the second and third floors.

Employees must be able to travel to all levels of the facility in a safe and reasonable time frame.Skills/Qualifications for the Forensic Psychiatric Nurse Practitioner

  • Part Time include:
  • NYS Psychiatric Nurse Practitioner. Board certification is preferred.
  • Has strong clinical skills and experience providing treatment to persons with serious mental illness; a minimum of three years of experience in psychiatry is required.
  • Must have a strong commitment to the rights and ability of each person with a mental illness to make own decisions and choose, live in the community, work, and have access to continuous support and services to enhance wellness.
  • Must be able to effectively work with all people and demonstrate skills and competence to establish supportive and trusting relationships with all individuals.
  • Must uphold individuals’ rights and personal preferences in treatment and treat everyone with respect and dignity.

Continuously acting as a guest in individuals’ homes and family homes, is courteous, polite and an active listener.

  • Valid and insurable driver's licenses. Must use your own vehicle for business.

Agency vehicles are available and need to be used to transport individuals.Salary and Schedule of the Forensic Psychiatric Nurse Practitioner

  • Part Time:
  • $120.96/hour.
  • The number of hours per week is negotiable with a Monday-Friday daytime schedule.
