Summary

Position Highlights:
The Advanced Clinical Pharmacist Specialist (ACPS) – Intensive Care Unit is a core member of the Acute Care Clinical Pharmacy Team, providing advanced, patient-centered pharmaceutical care to critically ill post-operative and medical ICU patients within a highly specialized oncology- focused institution. This role supports a 7 days on / 7 days off daytime schedule and is embedded directly within the intensive care environment, working alongside multidisciplinary teams to manage complex, high-risk
pharmacotherapy.

The ACPS practices at the top of licensure, serving as a recognized pharmacotherapy expert for critically ill patients with surgical, medical, and oncologic comorbidities, including those with severe infections, organ dysfunction, hemodynamic instability, and complications related to major oncologic surgeries. The position emphasizes direct bedside involvement, proactive medication optimization, and real-time clinical decision-making for high-acuity patients.


This role collaborates closely with intensivists, surgeons, advanced practice providers, nursing, respiratory therapy, nutrition, and case management to ensure safe, evidence-based, and individualized medication therapy throughout the ICU stay and during transitions of care. The ACPS plays a critical role in the management of high-risk medications, protocol-driven ICU therapies, and medication safety initiatives, enhancing patient outcomes and supporting institutional quality goals.


In addition to direct clinical care, the position contributes to the advancement of critical care pharmacy practice through education,
precepting of pharmacy learners, participation in interdisciplinary committees, and development of ICU-focused guidelines and protocols. This role is well-suited for a pharmacist who thrives in a collaborative environment and is committed to clinical excellence, professional
leadership, and continuous practice improvement in critical care.

 

The Ideal Candidate:

  • Doctor of Pharmacy Degree from an accredited College of Pharmacy or equivalent combination of education and experience.
  • Board of Pharmacy Specialties (BPS) certification in area of specialty practice.
  • Relevant work experience as a clinical pharmacist in the intensive care unit within an immunocompromised patient population.


Responsibilities:

  • Directs and optimizes advanced clinical pharmacy services for critically ill post-operative and medical ICU patients, including those with complex surgical oncology conditions, multisystem organ dysfunction, and immunocompromised states.
  • Round with interdisciplinary Critical Care teams including intensivists, advanced practice providers, respiratory therapists, nutritionists, case managers, and learners ranging from medical student to surgical Fellows to design, implement, and monitor patient-specific pharmacotherapeutic plans.
  • Provide pharmacotherapy recommendations for the non-critically ill neurosurgical patient population.
  • Precept APPE Pharmacy Students and PGY1 Pharmacy Residents as part of our ASHP-accredited Residency Program.
  • Participate in interdisciplinary clinical committees focused on quality, safety, and guideline development.


Credentials and Qualifications:

  • PharmD 
  • Licensed Pharmacist (RPh/PS)
  • Completion of a PGY-2 Pharmacy Residency in area of specialization OR PGY-1 residency AND a minimum of three (3) years of progressive experience as a clinical pharmacist in a comprehensive academic setting OR 8 years of progressive experience as a clinical pharmacist in a comprehensive academic setting with significant time practicing in applicable specialty area.
  • Board of Pharmacy Certification (BPS) in area of Specialty within 18 months of hire or transfer.
    • BPS certifications allowed are: BCPS - Board Certified Pharmacotherapy Specialty, BCOP - Board Certified Oncology Pharmacist, BCIDP - Board Certified Infectious Disease Pharmacist, BCPMP -
      Board Certified Pain Management Pharmacist, BCCCP - Board Certified Critical Care Pharmacist
