About the Role:
As an Experienced Home Health Caregiver with Modani Care, you will be responsible for providing high-quality, personalized care and support to our clients in their own homes. Your duties will include:
- Providing assistance with daily living activities, such as bathing, dressing, and grooming
- Managing medication regimens and administering medications as directed
- Assisting with mobility and transferring clients, including use of assistive devices
- Preparing meals and snacks, and ensuring proper nutrition and hydration
- Maintaining a clean and safe living environment for clients
Responsibilities:
- Develop strong relationships with clients and their families to understand their unique needs and preferences
- Collaborate with other Modani Care team members, including nurses and social workers, to ensure comprehensive care and support
- Maintain accurate and timely records of client care and services provided
- Participate in ongoing training and education to stay current with best practices and industry developments
Requirements:
- At least 2 years of experience in home health care or a related field
- High school diploma or equivalent required; post-secondary education in a healthcare-related field is an asset
- Current CPR/First Aid certification required; additional certifications (e.g., HHA, CNA) are an asset
