- Provide nursing care and support to program participants, including health assessments, treatment planning, and coordination of medical services.
- Collaborate with the ACT team to ensure participants receive integrated physical and mental health care.
- Conduct nursing assessments and develop individualized care plans
- Implement and evaluate nursing care
- Coordinate health services with the ACT team and other providers
- Lead group sessions on health, wellness, and recovery
- Educate staff on medical care and medication procedures
- Advocate for participants to access medical care and community supports
- Supervise LPNs on medication management
- Ensure participants have primary care providers and receive regular exams
- Administer medications, vaccines, and perform basic procedures
- Maintain accurate health records and documentation
- Provide culturally competent care and crisis intervention as needed
- Licensed and registered as a professional nurse in NYS
- BSN preferred
- Valid driver's license
- Bilingual (Spanish) desirable
- Experience with serious mental illness, forensic populations, co-occurring disorders, or HIV/AIDS preferred
- New grads welcome
