Support individuals diagnosed with mental illness, intellectual disabilities, or substance use disorders.
Advocate for clients and coordinate access to various services.
Develop personalized service plans to enhance clients' quality of life.
Engage in significant face-to-face interaction within the community.
Collaborate with multiple service providers to ensure comprehensive care.
Key Responsibilities
Engage with and advocate for individuals with mental illnesses.
Assist individuals in accessing mental health services, housing, medical care, education, and employment.
Develop individual service plans focusing on consumer strengths and goals.
Provide face-to-face contact with individuals and families in the community.
Coordinate service provisions through various programs and facilities.
Participate in treatment planning meetings.
Provide support and assist with crisis resolution.
Qualifications
Bachelor's degree in sociology, social welfare, psychology, or related social sciences.
Valid driver's license and ability to drive a company vehicle.
