Description
- We are seeking a professional for a full-time position as a Children's Blended Case Manager in the Philadelphia, PA area.
- This role is onsite and requires bilingual proficiency in Spanish and English.
- The position involves extensive case management services for high-risk children and adolescents, with a focus on crisis intervention and community resource connection.
- Provide case management services to approximately 25 high-risk children and adolescents.
- Conduct functional assessments to identify skill deficits and strengths.
- Develop and implement service plans based on assessments.
- Connect consumers with appropriate clinical and community resources.
- Maintain case management records and provide crisis intervention services.
- Participate in interagency and team meetings.
- Adhere to electronic health record and compliance policies.
- Bachelor's Degree in sociology, social work, psychology, gerontology, or related field.
- Two years of experience in human services with one year of direct client contact.
- Master's Degree preferred.
- Bilingual proficiency in Spanish and English required.
- Knowledge of crisis intervention and community resources in Philadelphia.
