RESPONSIBLE TO
Director of Missouri Slope at Home
PRIMARY JOB FUNCTION
The primary purpose of this position is to provide individual services for clients in their homes. These services may include, but are not limited to, personal cares, companionship, transportation, housekeeping, meal preparation, safety checks, chore services, and any other services the program provides for aging adults in their homes.
KNOWLEDGE, SKILLS & ABILITIES
- Possess excellent verbal, writing, and organization skills and are able to effectively work and communicate with a wide range of individuals from all socio-economic groups, including geriatric persons, people with special needs, and those in need of memory care support
- Able to work with minimal supervision and effectively manage workflow. Ability to make independent decisions when circumstances warrant such action. Able to perform all personal care duties, as well as light housekeeping, simple cooking, and basic home maintenance.
- Proficient with email, able to maintain required records on the database, utilize training software, and communicate through the use of technology.
Must possess certain basic skills, including the ability to:

ESSENTIAL JOB FUNCTIONS
- Services Providing:
Provide all assigned services for clients. These include, but are not limited to, personal care (e.g., bathing, dressing), companionship, transportation, housekeeping, meal preparation, safety checks, chore services, and any other services contracted by the client.
- Home Safety Checks/Checks
Ensure each client is contacted at their predetermined time. Identify any “out of the ordinary” indicators, whether all is well or not. Respond appropriately if something is off.
- Companion Services
Provide supervision and socialization to the client, which can include light housekeeping, laundry, shopping, and going out to events or dining.
- Meals
Prepare meals within the client's home or prepare meals from Missouri Slope.
OTHER JOB FUNCTIONS
- Personal Care:
Provide personal care/activities of daily living (ADLs) for clients who elect this Service (including dressing, bathing assistance, toileting, incontinence care, eating, oral care, exercises, compression stocking application, and shaving).
- Housekeeping/Homemaker Service:
Provide housekeeping and home-making services (housework, laundry, garbage removal, vacuuming, dusting, etc.) for clients who elect this service. Ensures the client has the appropriate products available to clean properly. Also includes shopping for clients’ essential needs
- Transportation:
Transport the client in either a personal or agency vehicle for non-medical tasks such as shopping or errands.
- Personal Emergency Response System:
If ERS is available, ensure the system is in working order, that the client has the pendant/watch on their person, and that it is charged before leaving each shift.
- Documentation:
Complete all documentation necessary for client services, shift check-in/check-out, training, and any other required paperwork by Missouri Slope at Home, utilizing Aanie/smart care app or hard copy forms

MINIMUM QUALIFICATIONS
- Applicant must be 18 years of age or older.
- Must have at a minimum a high school diploma or GED.
- Experience providing direct care assistance to older adults.
- Must be able to meet competency standards for QSP criteria in North Dakota (training provided by Missouri Slope).
- Must possess a valid driver’s license, maintain an acceptable driving record, and have reliable transportation to travel to and from clients’ homes and/or as needed based on operational and client needs. Proof of current vehicle insurance, in accordance with state licensing requirements, is required.
- Successfully pass a criminal background check, including motor vehicle record (MVR), drug screening, and reference check.
- Successfully complete all required training and/or certification within designated timeframes, which will vary depending on the specific program assigned. Employees in this position may be eligible to participate in a CNA training program through Missouri Slope. Eligibility is contingent upon job performance, including attendance, and upon the employee holding a full-time position.
- Successfully pass a drug test, background check, including criminal history, certification registries, and employment history
- Completion of training as required by Missouri Slope

PHYSICAL DEMANDS
Employees must be able to climb stairs; balance; stoop; kneel; crouch; reach above shoulder level; bend and twist; and sit, stand, and walk for extended periods. The role also requires the ability to push, pull, lift, grasp, perform fine motor tasks, communicate effectively (both speaking and hearing), and perform repetitive motions. Employees must occasionally lift and/or move objects weighing up to 50 pounds. The position may also involve assisting clients who require Hoyer transfers.
WORK CONDITIONS
- Services are provided in clients’ private residences under varying and sometimes unpredictable environmental conditions. Employees must be able to work independently and adapt to diverse home settings. Work is primarily conducted indoors, with occasional outdoor duties. Assigned work shifts and locations will vary depending on the customers’ and/ or agency needs.
- Employees may be exposed to pets and pet dander, tobacco smoke, unpleasant odors, fragrances (including air fresheners and candles), cleaning products, plants, dust, and other environmental allergens. Home environments may vary in cleanliness, lighting, space, and temperature.
- Work involves close physical proximity to clients, often providing hands-on care or working within inches of the client. This requires the use of personal protective equipment (PPE), including masks, gloves, and other protective attire as appropriate. There is potential exposure to hazardous materials, including bloodborne pathogens and household chemicals, requiring adherence to infection control protocols and basic chemical safety practices.
