Mission Hospice & Home Care serves patients and families in the San Francisco Peninsula and South Bay with quality care and compassionate end-of-life support and education. Founded in 1979, we are a local, independent nonprofit that has provided thousands of patients and their families with physical, emotional, and spiritual support throughout the journey from a life-threatening diagnosis through death and bereavement.

The DPCS is responsible for establishing, implementing, and evaluating the organization’s clinical services to meet and promote standards of quality and contribute to the organization’s mission and goals in compliance with organization policies and procedures and applicable laws and regulations.

Schedule

Monday - Friday; 8:30am - 5:00pm, AOC duties required occasionally

Location:

San Mateo with occasional travel to Pleasanton

By the Bay Health follows all CDPH vaccine requirements for healthcare personnel.

Essential Duties & Responsibilities include (but are not limited to):

  • Oversee hospice departments (clinical and/or non-clinical) as determined by the COO.
  • Coordinate and oversee all direct and indirect patient services provided by clinical personnel.
  • Provide guidance and counseling to team leaders/site manager/other supervisors to assist them in continually improving all aspects of hospice and palliative care services provided through agency personnel.
  • Assist clinical managers in managing hospice and palliative care teams and planning.
  • Analyze operational indicators to identify census changes and increases or decreases in volume that could impact staffing levels, revenues or expenses and recommend and implement as appropriate.
  • Collaborate with hospice and palliative care staff to provide effective coordination of services and evaluate delivery of care.
  • Prepare annual budget for assigned area of responsibility and monitor allocation of resources according to budgetary limitations.
  • Review and approve patient care services invoices in collaboration with the finance department.

Education/Experience

  • Bachelor's degree (BSN) in Nursing from an accredited school of nursing. Minimum of 3 years’ experience within the last 5 years in a hospice or home health agency, primary care clinic or health facility, at least one year of which was in a supervisory or administrative capacity
  • Or Associate’s degree (ADN) in Nursing from an accredited school of nursing. Minimum of 4 years’ experience within the last 5 years in a hospice or home health agency, primary care clinic or health facility, at least one year of which was in a supervisory or administrative capacity

Certificates, Licenses, Registrations

  • Current license as a registered professional nurse in the State of California, BSN preferred.
  • CHPN and/or PHN certification preferred

Pay Range

$160K - $175K annually
