The Inpatient Hospice Social Worker located at Stanford Hospital provides social work care to hospice patients who qualify for end-of-life care in a hospital setting in collaboration with the Stanford Hospital inpatient team that includes a Hospice RN and Chaplain. This social worker will be on-site at Stanford Hospital and requires a high degree of courtesy, tact, and knowledge of services.

Essential Duties & Responsibilities include (but are not limited to):
- Perform initial and ongoing hospice eligibility assessment and consultation with Hospice MD in collaboration with Standford and in accordance with CMS guidelines
- Assume primary responsibility for a patient/family using the nursing process to assess, plan, implement and evaluate patient/family needs, goals, and interventions.
- Perform evaluation and coordination of social work and spiritual support services needed by patient/family.
- Coordinate the implementation of the plan of care and ensure care coordination with the inpatient team.
- One year experience as a Social Worker.
- Experience in acute care/ home health and/or hospice setting preferred.
- Current California Driver’s License and automobile insurance that meets State of California requirements, if driving a car.
$44.00 - $51.25
