Responsibilities
- Perform comprehensive patient assessments, including vital signs, physical examinations, and detailed medical histories.
- Administer medications and injections accurately following prescribed protocols, including IV infusions and catheter care.
- Manage complex cases involving acute pain management, dementia care, Alzheimer's care, and infectious disease protocols.
- Operate electronic health record (EHR) systems such as Epic, Cerner, or Athenahealth for precise medical documentation and medical records management.
- Conduct diagnostic evaluations and coordinate discharge planning to ensure seamless transitions of care across settings.
- Provide specialized nursing interventions such as tube feeding management, ventilator support, and sterile processing techniques.
- Collaborate with multidisciplinary teams on case management, triage assessments, and emergency response procedures in high-pressure environments like Level I Trauma Centers or ICU units.
Qualifications
- Valid licensure as an LPN or RN with current state licensing requirements.
- Proven experience in skilled nursing facilities or hospital settings with exposure to medical-surgical, ICU, emergency medicine, or long-term care environments.
- Demonstrated proficiency in EMR/EHR systems such as Epic, Cerner, or eClinicalWorks; familiarity with ICD coding (ICD-9/10) is highly desirable.
- Strong knowledge of physiology, anatomy, pharmacology, and medical terminology relevant to diverse patient populations including geriatrics and pediatrics.
- Experience working with individuals with disabilities or developmental delays; familiarity with dementia and memory care is a plus.
- Ability to perform advanced clinical procedures such as venipuncture, phlebotomy, suturing, airway management, and first aid.
- Excellent communication skills for caregiver interaction and patient education; commitment to maintaining clinical confidentiality policies in accordance with HIPAA regulations.
