LPN/ RN in Skilled Nursing

Registered Nurse (RN) / Licensed Practical Nurse (LPN)

Position Summary

Mission is hiring part-time/ PRN for both Night or Dayshift

The RN/LPN provides compassionate, resident-centered nursing care in a skilled nursing and long-term care setting. This position is responsible for assessing resident needs, providing and coordinating care, supervising nursing assistants, communicating changes in condition, and ensuring care is delivered safely and in accordance with the resident’s plan of care, provider orders, facility policies, and applicable regulations.

Essential Duties & Responsibilities

• Provide direct nursing care to residents based on individual needs and plans of care.
• Complete nursing assessments, documentation, treatments, and medication administration accurately and timely.
• Monitor residents for changes in condition and promptly notify the provider, responsible party, and appropriate members of the interdisciplinary team.
• Initiate appropriate interventions and follow through to ensure resident needs and changes in condition are addressed.
• Supervise, support, and provide direction to CNAs throughout the shift.
• Ensure CNAs are completing assigned care, rounds, toileting, repositioning, ADLs, and other resident-specific interventions.
• Respond promptly to emergencies, falls, changes in condition, and other clinical concerns.
• Complete required post-fall assessments, neurological checks, incident documentation, and follow-up interventions as indicated.
• Administer medications and treatments according to provider orders and accepted nursing standards.
• Monitor effectiveness of medications, treatments, and interventions and report concerns appropriately.
• Maintain accurate, complete, and timely documentation in the medical record.
• Participate in admissions, discharges, transfers, care planning, and interdisciplinary communication.
• Follow infection prevention practices, isolation/precaution requirements, and Enhanced Barrier Precautions as applicable.
• Promote resident dignity, privacy, choice, independence, safety, and quality of life.
• Communicate professionally with residents, families, coworkers, providers, and outside healthcare partners.
• Provide leadership on the nursing unit and help maintain a positive culture of teamwork, accountability, and resident-centered care.
• Report concerns related to abuse, neglect, exploitation, misappropriation, safety, or quality of care immediately according to facility policy.
• Participate in staff education, quality improvement initiatives, survey readiness, and other duties as assigned.

Qualifications

• Current and unrestricted RN or LPN license in the state of practice.
• Current CPR/BLS certification as required.
• Skilled nursing, long-term care, rehabilitation, or geriatric nursing experience preferred but not required.
• Strong clinical judgment, communication, organization, and leadership skills.
• Ability to prioritize resident needs and respond appropriately to changes in condition.
• Ability to work effectively as part of an interdisciplinary team.
