What are the responsibilities and job description for the SUD Navigator position at MISIDE?
POSITION SUMMARY:

Serve as part of an integrated, comprehensive community mental health team supporting individuals with severe and persistent mental illness.

Provide social support, community resource referrals, and advocacy for patients and their families.

Deliver crisis management services as part of a comprehensive care management approach.

Provide direct clinical services, including individual, group, and family therapy.

Offer skill-building support to patients.

Assist patients in setting personal goals and applying recovery principles.

Conduct ongoing follow-up, basic motivational interviewing, and goal setting.

Provide therapy for patients with behavioral health issues complicated by environmental or personal challenges.

Fully integrate into the patient care team and actively participate in care planning.

Identify and communicate psychosocial issues that may complicate patients’ health conditions to medical staff.
MINIMUM JOB REQUIREMENTS:
- Bachelor’s degree in related field.
- Registration or certification from a recognized certifying organization, agency or Board evidencing training and expertise in areas appropriate to SUD services (CAADAC, CCAPP, CADTP, CADDE, CARR)
- Minimum 2 years of experience providing SUD services in a community and/or outpatient setting
