Responsibilities:
- Provide skilled nursing care to residents, including medication administration, wound care, and mobility assistance
- Develop and implement individualized care plans in collaboration with the interdisciplinary team
- Monitor and report changes in resident condition to the Charge Nurse and other healthcare professionals
- Participate in quality patient assessment, planning, and evaluation
- Collaborate with physical, occupational, and speech therapists to develop and implement treatment plans
- Provide emotional support and advocacy to residents and their families
- Current licensure as a Licensed Practical Nurse (LPN) in the State of Florida
- 1 year of experience as an LPN
- Strong communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
