- Monday - Friday 8 am – 6 pm
- Saturday - 8 am to 4 pm
Essential Duties & Responsibilities
- Provide direct patient care in accordance with nursing standards, clinic protocols, and provider instructions.
- Obtain and document vital signs, medical history, chief complaint, medication lists, and other intake information.
- Assist providers with examinations, procedures, wound care, immunizations, point‑of‑care testing, and diagnostic collection (e.g., urine, strep, flu, COVID‑19 swabs).
- Administer medications per provider orders, including oral medications and injections.
- Perform clinical tasks such as EKGs, nebulizer treatments, pulse oximetry, specimen handling, and CLIA‑waived testing.
- Support chronic care management by providing patient education, medication reconciliation, and follow‑up outreach.
- Maintain accurate, timely documentation in the electronic health record (EHR).
- Participate in care coordination, referrals, and communication with specialists, hospitals, and ancillary services.
- Reinforce infection control, safety practices, and compliance with MHHS policies, HIPAA, and regulatory guidelines.
- Maintain exam rooms, supplies, and medical equipment to ensure cleanliness, readiness, and proper functioning.
- Support clinic operations by assisting with triage, scheduling needs, patient phone calls, and workflow optimization.
Qualifications:
- Education: Graduate of an accredited Practical Nursing program.
- Licensure: Current, unrestricted LPN license in the State of West Virginia.
- Certifications: BLS/CPR certification required (or obtained within 30 days of hire).
- Experience:
- Clinic, family practice, or ambulatory care experience preferred.
- New graduates are encouraged to apply.
- Skills:
- Strong assessment, communication, and clinical documentation skills.
- Ability to multi‑task in a fast-paced rural clinic environment.
- Proficiency with computers and electronic health records (Epic, Athena, or similar).
- Demonstrated patient‑centered approach and excellent customer service.
