General Definition of Work

The Promotor(a) de Salud/Bilingual Family Support Specialist performs intermediate county services work serving as a trusted liaison between the Hispanic community, partner organizations, and County departments. The position promotes equitable access to health and human services through culturally responsive outreach, education, advocacy, resource navigation, and care coordination. Work involves building relationships with Spanish-speaking individuals and families, connecting them to appropriate services and resources, collaborating with County departments and community partners to reduce barriers to access, and supporting improved health and well-being. Work is performed under the general supervision of the assigned supervisor.

Essential Functions

Conduct outreach and build effective relationships with individuals, families, and community partners to increase access to services and promote community well-being of the Hispanic community.

Assess Spanish-speaking client and community needs, identify barriers to accessing services, and connect individuals with appropriate county, community, healthcare, and public assistance resources.

Provides translation and interpretation services, verbal and written, to support effective communication across all County departments, ensuring equitable access to County programs, services, and information for individuals with limited English proficiency and other language access needs.

Provide care coordination, resource navigation, referrals, and follow-up to support successful access to services and improve outcomes.

Deliver culturally responsive education, information, and support related to health, wellness, human services, and available community resources.

Advocate for equitable access to services, empower individuals to navigate systems independently, and support informed decision-making.

Collaborate with internal staff, healthcare providers, collaborative partners, schools, nonprofit organizations, government agencies, and other community partners to coordinate services and address community needs.

Assist Public Health & Human Services with interpreting services for in-home and office visits including MN Choices Assessments, immunizations, lactation appointments, scheduling appointments, and assisting individuals with financial assistance applications.

Assist Child Services unit with facilitating supervised visitation and transportation, provide culturally and linguistically appropriate education.

Participate in community engagement, program development, and interdisciplinary initiatives that strengthen service delivery and address emerging issues.

Track outreach activities and program outcomes. Maintain accurate documentation, records, reports, and data in accordance with applicable laws, regulations, and organizational policies, including confidentiality requirements.

Travel throughout the county and work in a variety of settings, including county offices, client homes, healthcare facilities, schools, and community locations, to provide services and support including interpreting services.

Comply with all applicable federal, state, and local laws, regulations, policies, and departmental procedures governing the delivery of human services.

Provide services in a professional, culturally responsive, impartial, and ethical manner, including accurate communication and interpretation when applicable.

Identify, document, and report suspected fraud, abuse, misuse of benefits, or other program integrity concerns in accordance with departmental policies and procedures.

Performs the physical and mental demands of the position.

Attends work during regularly scheduled hours including occasional evening or weekends hours for events.

Other related duties as assigned.

Knowledge, Skills and Abilities

Knowledge of county service programs, community resources, health equity, social determinants of health, community engagement strategies, culturally responsive practices, and principles of trauma-informed care and motivational interviewing. Ability to communicate fluently in English and Spanish, including accurately interpreting and translating information and effectively communicating with individuals with limited English proficiency. Ability to establish trusting relationships with diverse populations; communicate effectively with individuals from varied cultural, linguistic, and educational backgrounds; work independently and collaboratively within interdisciplinary teams; maintain confidentiality; organize and prioritize work; utilize computer software and electronic record systems; and navigate sensitive or difficult situations with empathy, professionalism, and sound judgment.

Education and Experience

High school diploma or equivalent required.

Associate degree in Human Services, Public Health, Community Health, Social Work, or a closely related field preferred.

Experience working with diverse populations and individuals experiencing social, economic, behavioral, or health-related barriers preferred.

Experience in community outreach, health education, case coordination, resource navigation, or the delivery of human services preferred.

An equivalent combination of education and experience that provides the required knowledge, skills, and abilities may be considered.

Physical Requirements

This work requires the occasional exertion of up to 20 pounds of force to lift, carry, push, pull, or otherwise move objects. Work regularly requires standing, walking, sitting, speaking, hearing, using hands to finger, handle, or feel, and performing repetitive motions; frequently requires reaching, bending, and operating a motor vehicle; and occasionally requires climbing, balancing, stooping, crouching, or lifting. Standard visual acuity is required for reading documents, using electronic devices, and observing surroundings. Vocal communication and hearing are required to exchange information, provide instruction, and interact effectively with clients and community partners. Work involves preparing and maintaining written and electronic records, operating computers and other office equipment, and driving throughout the county to provide services. Employees perform work in a variety of settings, including county offices, client homes, healthcare facilities, schools, community organizations, and outdoor locations. The position may involve exposure to varying weather conditions, communicable diseases, bloodborne pathogens, domestic animals, and other environmental conditions encountered during field visits. Appropriate personal protective equipment may be required. Work is performed in both office and community environments with varying noise levels.

Special Requirements

Driver’s license valid to drive in the State of Minnesota.

Bilingual proficiency in English and Spanish.

Other Requirements

The incumbent may encounter not public data in the course of these duties. Any access to not public data should be strictly limited to accessing the data that are necessary to fulfill the employment responsibility. While data are being accessed, incumbent should take reasonable measures to ensure the not public data are not accessed by individuals without a work reason. Once the work reason to access the data is reasonably finished, incumbent must properly store the not public data according to the provisions Ch. 13. If a new work assignment requires access to not public data, the incumbent is permitted to access not public data for the work assignment purposes only. Any access to not public data must be strictly limited to the data necessary to complete the work assignment and after the assignment is completed, the employee’s work assignment no longer requires access.

Salary/Benefits: Starting wage is $25.79 per hour per hour plus health insurance, life insurance, retirement plan, vacation/sick/holiday pay and many other voluntary benefits.  

Salary range is $25.79- $36.46
