Job Summary
We are seeking a dedicated and skilled Physical Therapist (PT) to join our team. The ideal candidate will have a strong background in acute care, geriatrics, and home health settings. This role involves assessing patients' physical conditions, developing personalized treatment plans, and implementing effective therapy techniques to improve mobility and quality of life. The PT will work collaboratively with healthcare professionals to ensure comprehensive patient care.
Duties
- Conduct thorough evaluations of patients' physical abilities and limitations.
- Develop and implement individualized care plans based on patient assessments.
- Utilize various therapeutic techniques, including trigger point therapy, to address specific patient needs.
- Provide education and support to patients and their families regarding treatment options and home community care strategies.
- Collaborate with interdisciplinary teams to coordinate discharge planning and ensure continuity of care post-treatment.
- Maintain accurate documentation of patient progress and treatment outcomes in compliance with clinic standards.
- Stay updated on the latest developments in physiotherapy practices and integrate new knowledge into patient care.
Experience
- Previous experience in acute care, geriatrics, or home health settings is preferred.
- Strong knowledge of human physiology and rehabilitation systems is essential.
- Familiarity with developing care plans tailored to individual patient needs is highly desirable.
- Experience in a clinical environment is advantageous, particularly in managing diverse patient populations.
