Position Summary:
The Client Care Coordinator serves as a vital link between medical providers, external professionals, and in-house behavioral health teams. This role ensures seamless communication and collaboration to support comprehensive client care.

The coordinator engages clients throughout their treatment journey at Mindful Health by offering timely support, education, and guidance. They are responsible for providing same-day care when needed, particularly for clients initially visiting behavioral health appointments. This includes reviewing therapeutic skills, care plans, and connecting clients with appropriate resources.

Overall, the Client Care Coordinator helps foster continuity of care, addresses client questions or concerns, and supports clients in staying actively involved in their wellness.

Education & Experience Requirements:
  • High school diploma required; Associate degree in a related field preferred.
  • Minimum of one year of experience in a behavioral health facility, healthcare practice, or similar setting (or equivalent combination of education and experience).
  • At least one year of experience working with insurance processes or coverage verification preferred.
  • Familiarity with medical terminology is a plus.
  • Active CPR certification is required.
 
Key Responsibilities:
  • Receive and engage clients with identified behavioral health needs, review recommended care plans, and educate them on treatment options.
  • Connect clients with relevant Mindful Health services and resources to ensure continuity of care.
  • Clearly explain recommended levels of care, including cost, time commitment, and expectations.
  • Establish and manage payment plans for clients participating in services, when needed.
  • Serve as a liaison between clients and their treatment teams, proactively addressing barriers to care.
  • Act as the primary point of contact for referral sources, providing timely and accurate information.
  • Deliver exceptional customer service by practicing active listening, show empathy, and communicating with clarity and professionalism.
  • Demonstrate cultural sensitivity and individual awareness when addressing client needs.
  • Ensure efficient handling of client inquiries and follow established escalation protocols when necessary.
  • Maintain full compliance with Mindful Health policies and HIPAA regulations.
  • Support additional administrative or client service duties as assigned.

Physical Requirements:
  • Ability to alternate between sitting and standing for extended periods.
  • Capable of lifting, holding, pushing, and pulling up to 15 pounds.
  • Proficient in operating a computer or laptop, including extended periods of typing, clicking, and monitor use.
  • Adept visual capabilities including close vision, color distinction, depth perception, and the ability to adjust focus with strong hand-eye coordination.
  • Effective verbal and written communication skills to interact with clients, team members, and customers.
  • Ability to read, write, and comprehend information through active listening.
  • Able to consistently, safely, and successfully perform essential job functions in compliance with ADA, FMLA, and applicable federal, state, and local standards, including productivity expectations.
  • Dependable attendance and punctuality in alignment with regulatory and organizational standards.

Mindful Health offers:

  • Schedule: Monday through Friday, 8 am – 5 pm
  • Salary: $23.00 - $26.00/hour

  • 40 hours (5 days) of paid sick leave
