Position Summary

In this role, you will assess and treat speech, language, cognitive-communication, and swallowing disorders, creating individualized therapy plans to help patients improve communication, swallowing, and overall quality of life.
Key Responsibilities
- Assess patients in their homes and create individualized therapy plans.
- Provide skilled interventions for speech, language, cognition, and swallowing.
- Educate patients and caregivers on strategies to improve communication and daily functioning.
- Document progress and ensure compliance with Medicare Part B regulations.
- Collaborate with the home health/ therapy care team to coordinate patient care.
- Adjust treatments based on patient progress and needs.
- Active Speech-Language Pathology (SLP) license in the state of practice.
- Certificate of Clinical Competence (CCC-SLP) required.
- Experience in home health or outpatient therapy preferred.
- Knowledge of Medicare Part B documentation and regulations.
- Strong assessment, treatment planning, and patient education skills.
- Excellent communication and interpersonal abilities.
- Reliable transportation and a valid driver's license.
- Comfortable working independently and managing a flexible, home-based schedule.
- Proficiency with electronic medical records (EMR) and basic computer skills.
- CPR and other relevant certifications as required.
