Position Summary
We are seeking a Licensed Clinical Social Worker (LCSW) to provide in-home services for Medicare Part B patients. In this role, you will assess psychosocial needs, develop care plans, provide counseling and resource coordination, and support patients and their families in managing emotional, social, and environmental challenges.
Key Responsibilities
- Assess patients' psychosocial needs and develop individualized care plans.
- Provide counseling and emotional support to patients and families in the home setting.
- Assist patients in accessing community resources, support services, and benefits.
- Collaborate with the home health/ therapy care team to coordinate care.
- Document assessments, interventions, and progress in compliance with Medicare Part B regulations.
- Educate patients and families on coping strategies, mental health management, and available resources.
- Advocate for patient rights and ensure care plans align with patient goals and needs.
- Adjust interventions as needed based on patient progress and changing circumstances.
- Active LCSW license in the state of practice.
- Master's degree in Social Work from an accredited program.
- Experience in home health, medical social work, or geriatric care preferred.
- Knowledge of Medicare Part B documentation and regulations.
- Strong assessment, counseling, and crisis intervention skills.
- Excellent communication, organization, and problem-solving abilities.
- Reliable transportation and a valid driver's license.
- Comfortable working independently and managing a flexible, home-based schedule.
- CPR and other relevant certifications as required.
