Position Summary

The Certified Assessor completes comprehensive assessments and planning services for people who are requesting long-term services and supports.

Essential Functions/Responsibilities

Duties listed are normal for this position.  These are not to be construed as exclusive or all-inclusive.  Other duties may be required and assigned.

Position Duties

  • Demonstrate best practices in assessment and support planning, using a person-centered approach, providing and explaining information, facilitating access to services, and following-up to assure people get connected.
  • Assure people have information about needed health care programs by sharing information about medical assistance and other health care programs, facilitating State Medical Review Team disability certification (if indicated), and informing people about long term and home and community-based services and supports, programs and service providers.
  • Determine eligibility for home and community-based programs and long-term services, whether state plan services can meet the person’s assessed needs and supports, the person’s level of care, case mix and home care ratings, provide guidance to people in selecting services and recommend referrals to other professionals and agencies as needed for cost-effective long-term services and supports.
  • Complete a Community Support Plan with each assessment and, for individuals who will receive state plan PCA/CFSS and Private Duty Nursing services, a Coordinated Services and Supports Plan, with a letter that includes your recommendations for the individual and case manager.
  • Collaborate as needed to provide an effective transition of the assessment to a case manager.
  • Coordinate with others to assure transition assistance to individuals residing in a nursing facility, hospital, regional treatment center or licensed supervised living facility who request or are referred for assistance.
  • Manage any appeals to level of care and eligibility determinations related to assessments you’ve completed.
  • Participate in the county’s multidisciplinary long-term care consultation/certified assessor team.
  • Authorize and enter PCA services and rates in the Medicaid Management Information System (MMIS).
  • Maintain up to date information regarding DHS regulations, rules, and procedures.

  • Ability to effectively apply casework knowledge and skills.

Minimum Qualifications

Education and Experience

  • Bachelor’s degree in social work or related field and at least one year of home and community-based experience.
  • Or Nursing Degree with certificate in public health nursing or closely related field and at least one year of home and community-based experience.
  • Or current license as a registered nurse and at least two years of home and community-based experience.
  • Or an equivalent combination of education and experience sufficient to perform the essential functions of the job.

 Required Training

  •  Minnesota Certified Assessor Training (MNCAT), initially and every 3 years thereafter. (training provided if not already completed)
  •  Training on Person Centered Thinking and Planning. (training provided if not already completed)
  •  Mandated Reporter Training. (training provided if not already completed)

 Required Certifications/Licenses

  • MnCHOICES Assessor Certification. (Training provided if not already completed)
  • Valid Minnesota driver’s license with proof of insurance.

Relationships

  • Regular interaction with County and Public Health program leaders.
  • Regular interaction with individuals referred for long term services and supports and their families and/or legal guardians to provide information about services, options, providers, and resources and gather information.
  • Interaction with service providers for referrals, planning, service coordination, and assuring the needs, health and safety of individuals are met/assured.
  • Interaction with medical, psychiatric, psychological, legal, educational, vocational, rehabilitation and other professionals and resources for referrals, information on services, and consultation.

 Supervisory Responsibilities

  • None.

 Working Conditions

  • Work is mainly performed in a standard office setting but may also be in clients’ homes, residential facilities, and other county offices.
  • May require working with persons who may be disoriented, hostile, aggressive, abusive, or threatening.
  • There may be hours worked outside of the normal business hours.

Physical Requirements Of This Position

  • Desk work involving use of a computer, telephone, and other devices.
  • Communicate effectively with County staff, clients, and professional groups.
  • Ability to read screens and documents.
  • Ability to drive in various light and weather conditions.
  • Ability to type and use a computer or mobile device.
  • Office work may include reaching for documents, grasping documents, standing, walking, and repetitive motions.
  • Lift and carry files and material weighing up to 20 pounds.
