Job Description
This position is responsible for identifying, assessing and care planning the psychosocial needs of the beneficiary and/or authorized representative to provide the necessary support, education, and advocacy to obtain care services and community resources. In addition, this position will coordinate the beneficiary’s safe transition from Community Medical Providers (a skilled nursing facility (SNF), long term acute care setting, acute rehab or hospital) to home. This position works in collaboration with the beneficiary/authorized representative, as well as the integrated and interdisciplinary healthcare team consisting of the Primary Case Manager and other members of the healthcare team as appropriate.
Qualifications
Master's degree in Social Work required.
Licensed in the State in Minnesota as a social worker (LSW).
4 years social work experience
3 years exp working with elderly/Adult populations
1 year of CM exp
Background working with Depression (add to resume)
