Responsibilities
Perform patient evaluations and develop individualized treatment plans based on physician orders
Deliver occupational therapy treatments and adjust plans as needed to achieve functional goals
Supervise COTAs, rehab techs, and therapy students as required
Maintain accurate and timely documentation in accordance with regulatory standards
Collaborate with interdisciplinary team members, including nurses, physicians, and physical therapists
Educate residents, families, and staff on therapeutic techniques and home exercise programs
Participate in discharge planning and facility quality improvement initiatives
Qualifications
Valid state licensure as a Physical Therapist
Prior experience in long-term care is a plus
Strong communication and organziational skills
Passion for working with geriatric population
