Responsibilities
- Assist residents or clients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support using patient transfer methods such as Hoyer lifts or manual handling techniques.
- Monitor patients’ vital signs and observe for any changes in health status, reporting findings accurately while adhering to HIPAA privacy regulations.
- Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption to promote proper nutrition.
- Support medication administration following care plans and ensure proper documentation of medication schedules and patient responses.
- Provide companionship and emotional support while engaging clients in meaningful activities that enhance mental health and social well-being.
- Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning patient areas, laundry, and organizing supplies.
- Assist with transportation to medical appointments or community activities using personal transportation or agency-provided resources.
Skills
- Proven experience working with individuals with disabilities, developmental or intellectual disabilities support, dementia care, Alzheimer’s care, or memory care environments.
- Knowledge of patient monitoring techniques, first aid procedures, CPR certification, and emergency response protocols.
- Familiarity with care plans development and implementation tailored to individual needs across home health, hospice care, geriatrics, or long-term care settings.
- Ability to perform heavy lifting safely using proper manual handling techniques; experience with patient transfer methods such as Hoyer lifts is preferred.
- Strong communication skills for effective client interaction and collaboration with healthcare teams while maintaining confidentiality per HIPAA standards.
- Experience working with individuals with autism or neurodevelopmental disorders is highly valued.
- Competence in assisting with personal transportation needs and supporting residents’ participation in community activities.
